In a concierge practice members pay an annual fee for services not covered by insurance or Medicare. Those services are possible because the annual fee allows Access MD to have a few hundred patients rather than the traditional primary care practice which has thousands of patients for each doctor. Those services include direct access to Dr. Hashimi, same day or next day appointments, the Doctor’s email and cell phone number, and may more benefits. This practice model allows Dr. Hashimi the time to listen to her patients, and gives the member a sense of security. Access MD patients benefit from direct attention from their physician who knows their complete medical history and unique medical needs. Patients value the prompt attention of their personal physician and the privacy of their medical visits. Our patient’s time is valuable, so they appreciate the scheduling benefits of direct access, on-time appointments, quick response, and expedited results. We can provide this level of service due to the structure of our practice.If your Access MD physician is out-of- town, appropriate arrangements will be made for you to be seen by another physician or a physician assistant may handle your problem over the phone. An appointment will be scheduled for you upon Dr. Hashimi’s return at the earliest possible time. Access MD patients are provided a private phone number answered by Dr. Hashimi or her nurse between 8 a.m. and 9 p.m., seven days a week. In most cases, the Access MD team will answer weekend calls. House calls may be available to established patients if medically necessary and as an ancillary service requiring an additional fee. If you have appointments in medical facilities outside of Mobile, Dr. Hashimi can accompany you at an additional cost outside of the normal annual fee. We find this is rarely necessary given today’s technology. Whether you are traveling for business or pleasure, Access MD will access the medical requirements and risks associated with your destination(s) and develop a personal healthcare travel plan including prescription drug preparation. Please schedule an appointment well in advance of your departure date so we can thoroughly evaluate your needs. Some vaccinations required for international travel can take six months to complete. Please call 911 if you have a life threatening situation. Whenever practical, we recommend that you request to be transferred to Springhill Hospital. Then have a family member notify Access MD via Dr. Hashimi’s private number of an emergency in progress. If indicated, Dr. Hashimi will meet you at the Emergency Room. Yes, most insurance company contracts require a co-payment for each visit. The co-payment for your initial visit will be covered by your annual fee as procedures are run during that initial visit which are not covered normally by insurance or Medicare. Our staff will make an appointment for you with a specialist in our network, or if you wish, you can make the appointment yourself. Access MD will expedite the transfer of your relevant medical records to the specialist and then integrate any new medical information into your comprehensive plan. The annual fee may be paid through your employer cafeteria plans or health savings account. Reimbursement through a flexible spending account or medical savings account may also be available. Please check with your benefits administrator or your tax advisor. Access MD will provide a statement for your records at the time of your annual physical exam or afterwards upon request. Yes, some of your annual fees can be covered by your HSA or FSA account, but please check with your accountant for more information. Yes, you are required to have insurance or Medicare. Fees are for services and amenities not covered by most insurance or by traditional Medicare plans. You will still need insurance for office visits, hospital stays, visits to specialists and any diagnostic testing. Access MD will submit any eligible charges for office visits to your insurance or Medicare. Please note that unfortunately we are not accepting new patients under the Medicare “Advantage” programs. Yes, you may cancel at any time. Any refunds granted will be prorated based on the number of months of service in the given year less the value of specific concierge services rendered. The amount of refund will be at the discretion of the doctor. If you move or decide to leave the practice, we will assist your transfer of records in accordance to AMA and state guidelines. As a benefit, you will not be charged a fee for your medical records. Simply call us at 251-895-4345 to set up a no-obligation appointment to see the office and meet with Dr. Hashimi.
